A specialist support service offering one-to-one support for people in their own homes.

The Support at Home service is provided by staff who have been trained in Dementia care and understand how important it is for everyone to maintain their independence.

Support Workers can accompany on outings, shopping, or visits; help to manage bills and correspondence or help with any activities.

The service is available for a minimum of 2 hours a week or as long as required.

"This service was launched in May 2011 offering a domiciliary service (personal care) to clients in their own homes.

In addition to assistance with washing, dressing and medication we can also offer help with household tasks such as preparation of meals and drinks plus emotional support.

Each client is assigned a small team of trained care workers, one or two of whom will be the client’s main care workers.

The service is for a minimum of one hour in order to deliver an unhurried quality visit."

Carers First – Kent Carers’ Emergency Card

,

One of the main concerns that carers tell us they have, is when leaving the person they look after, what would happen if they were taken ill or involved in an accident.

Carers are issued with a credit card sized card. The carer carries it at all times, so that it can be used as an instant source of identification in case of accident or sudden illness.

The card does not have any personal details on it, only a unique registration number and the telephone number of the central help line.
In an emergency situation anyone can telephone the number on the card and quote the unique registration number printed on the card. The contact centre uses the registration number to easily access the carers pre-arranged emergency plan and puts it into action.

This is a free service.

"Dial 2 Drive (D2D) is a community transport scheme for people unable to access public transport due to older age, illness or disability living in Sevenoaks, Tonbridge, Tunbridge Wells and the surrounding areas.

Volunteer drivers use their own cars to take clients to medical and social appointments. This includes appointments at local surgeries, hospitals, day centres, hairdressers, opticians, lunch clubs and shopping.

There is an annual membership fee of £20.00. The drives are charged at 65p per mile from the driver's home to completion of journey (driver returning home). We will always try to find a driver who lives closest to the client to keep the costs as low as possible. Volunteer drivers are only expected to wait up to an hour. If your appointment is much longer, you will need to book two separate drives (“double journey”)
We endeavour to keep costs as low as possible and do not make a profit from the scheme."

Sevenoaks District Council – Promoting Independence

The Council offers a range of services and advice for older people and those with disabilities...which helps to keep people living safely in their own homes for as long as they want to.

It may be able to provide grants up to £30,000, to home owners and those who rent privately or through a housing association, for property adaptations and improvements to help disabled people to continue to live independent lives. Funding is provided on a first come, first served basis.

If you don't meet the criteria for Council Funding, it can sign post and advise on accessing other ways to fund these types of alterations.

Advice can also be offered on other matters including personal alarm systems (Lifeline), welfare benefits, general housing concerns, energy efficiency.

Carers First – Kent Carers’ Emergency Card

,

One of the main concerns that carers tell us they have, is when leaving the person they look after, what would happen if they were taken ill or involved in an accident.

Carers are issued with a credit card sized card. The carer carries it at all times, so that it can be used as an instant source of identification in case of accident or sudden illness.

The card does not have any personal details on it, only a unique registration number and the telephone number of the central help line.
In an emergency situation anyone can telephone the number on the card and quote the unique registration number printed on the card. The contact centre uses the registration number to easily access the carers pre-arranged emergency plan and puts it into action.

This is a free service.

"Dial 2 Drive (D2D) is a community transport scheme for people unable to access public transport due to older age, illness or disability living in Sevenoaks, Tonbridge, Tunbridge Wells and the surrounding areas.

Volunteer drivers use their own cars to take clients to medical and social appointments. This includes appointments at local surgeries, hospitals, day centres, hairdressers, opticians, lunch clubs and shopping.

There is an annual membership fee of £20.00. The drives are charged at 65p per mile from the driver's home to completion of journey (driver returning home). We will always try to find a driver who lives closest to the client to keep the costs as low as possible. Volunteer drivers are only expected to wait up to an hour. If your appointment is much longer, you will need to book two separate drives (“double journey”)
We endeavour to keep costs as low as possible and do not make a profit from the scheme."

Carers First – Kent Carers’ Emergency Card

,

One of the main concerns that carers tell us they have, is when leaving the person they look after, what would happen if they were taken ill or involved in an accident.

Carers are issued with a credit card sized card. The carer carries it at all times, so that it can be used as an instant source of identification in case of accident or sudden illness.

The card does not have any personal details on it, only a unique registration number and the telephone number of the central help line.
In an emergency situation anyone can telephone the number on the card and quote the unique registration number printed on the card. The contact centre uses the registration number to easily access the carers pre-arranged emergency plan and puts it into action.

This is a free service.

Sevenoaks District Council – Promoting Independence

The Council offers a range of services and advice for older people and those with disabilities...which helps to keep people living safely in their own homes for as long as they want to.

It may be able to provide grants up to £30,000, to home owners and those who rent privately or through a housing association, for property adaptations and improvements to help disabled people to continue to live independent lives. Funding is provided on a first come, first served basis.

If you don't meet the criteria for Council Funding, it can sign post and advise on accessing other ways to fund these types of alterations.

Advice can also be offered on other matters including personal alarm systems (Lifeline), welfare benefits, general housing concerns, energy efficiency.

I am particularly pleased that our monthly meetings have included presentations and displays by individual groups. It is always interesting to discover what other groups have been doing.
